OUR MISSION


Jasmine Healthcare was founded by Chris Clark after he gained first-hand experience of care homes through personal family experience, where he had found the standards of accommodation and care offered to be poor. As a result, Chris passionately believes that many care homes for the elderly in England need to be improved. Consequently, Jasmine’s mission is:

OUR HOMES


Our homes are all original period properties with character, at the heart of the local communities which they serve. We endeavour to retain as many of the original features as we can, whilst sympathetically extending, modernising and refurbishing the buildings, to enhance the standard of living for all our residents.

This helps us to achieve, what we believe is the optimum size for a care home (40-50 residents), in terms of balancing the culture and community offered by them, with the facilities they offer. We also ensure that all of our homes, offer attractive private grounds, for the use of our residents and their visitors.

OUR AIMS


The specific aims of our mission are as follows:
  • For all our homes to have a team turnover rate of less than 30% per year
  • For over 50% of all our team to have a Diploma in Care Qualification
  • For all our homes to be rated Good or Outstanding by CQC and all other Regulatory bodies
  • For all our homes to have a Jasmine Compliance Score of over 85% at all times
  • For all our homes to have a www.carehomes.co.uk Rating of over 9.5 out of 10
  • For all our homes to be over 95% occupied, so that as many residents as possible can enjoy our high standards of care

This week is Dementia Awareness Week 💚

To help raise awareness of dementia, we'll be sharing information throughout the week about what dementia is, how to spot signs of dementia, the role person centred care plays in dementia care, and where to find support - including at Jasmine!

A huge thank you to our very own Care Assistant, Lyle Colyn, who put together this vital information as he continues to be a fantastic advocate for dementia care.

What are the early signs of Dementia? 🤔

Memory Difficulties: Forgetting recent events or conversions, and asking the same questions repeatedly.

Planning or Solving Problems: Finding it harder to manage bills, follow recipes or keep track of dates and appointments.

Finding the right words: Struggling to find the right words during conversations or losing track of what they are saying.

Getting lost or confused: Feeling unsure in familiar places or forgetting how to get somewhere they know well.

Changes in mood or behaviour: Feeing more anxious, worried, withdrawn, irritable or less interested in usual activities.

This week is Dementia Awareness week 💚

What is Person-Centred Care? 🤔

Person-Centred Care is about focusing care on the needs of the individual. To provide the best care, our care teams at Jasmine always make sure that people's preferences, needs and values guide the care that is provided, and that their care is always respectful of and responsive to them.

Person-Centred Care includes:

One-to-Ones: Discussions with care providers (usually over a cup of tea!) to understand the history, lifestyle, and preferences of the person receiving care. This could range from their favourite hobbies to the food they dislike!

Personal Care Plans: Care plans outline the care and support an individual needs to maintain their health, well-being, and daily functioning. For dementia patients, personalised care plans will be built around the care and support a person needs, with the family, carers and the individual themselves involved in putting together the plan.

Activities: Our Activities Coordinators across all our homes put together a weekly activity scheduled tailored to the needs of our residents, including those with dementia. A wide range of activities ensures all residents' need are met and everyone can get involved if they choose.
